Durable medical equipment and most medications related to the terminal illness are generally covered and offered at no cost to the patient or family. Sonata Hospice is accredited by the Joint Commission, and licensed by the state of California and a federally certified Medicare program.

Routine Level of Care

This type of hospice care is provided in the patient’s home and includes: assisted living communities, nursing homes, and long-term care facilities.

The Hospice Team

Sonata Hopsice TeamThe Primary Caregiver is identified by a family member or the patient and is responsible for the day-to-day care of the patient, while being supported and guided by the hospice team.

Medical Directors - Hospice Physicians are experts in pain and symptom management. Working alongside the patient’s attending physician, they provide oversight in all aspects of the hospice medical care.

Registered Nurses who are specialists in hospice care and experts in pain management are assigned to each patient. They are available around the clock to assist with any concerns that may arise.

Certified Home Health Aides are specially trained and available to assist patients with housekeeping duties such as laundry and meal preparation as well as bathing, shaving, shampooing and linen changes.

Medical Social Workers are advocates for the patient and the family. Each patient is assigned an experienced medical social worker who assists with the utilization of community resources, issues concerning insurance facility forms, financial matters, funeral arrangements, and placement decisions.

Chaplains provide non-denominational, faith support. As members of the healthcare team, they provide spiritual and bereavement support to patients, families, and staff members.
